Clinical Lead for the National Polypharmacy programme
Clare is a Senior Pharmacist with a background in clinical leadership of large, cross sector change at both local and national level. Her 30-year career has focused on medication safety, person centred care and improving prescribing especially in primary care. Clare is the clinical lead for the Health Innovation Polypharmacy Programme and works locally with Health Innovation Wessex. Clare is currently working with the RCGP and Royal Pharmaceutical Society looking at repeat prescribing systems.

What is the purpose of the network?
To provide a space for those working in General Practice to come together to communicate, share best practice, access training opportunities, discuss “wicked issues” and reduce duplication of work. The network space will continue to be developed by its members.
Who is the meeting for?
All those that work with and in the General Practice environment such as practice staff, PCN managers, ICB Patient Safety leads etc.
