UCLPartners Proactive Care Frameworks risk stratification searches for long term conditions.

The Frameworks support primary care teams to take a population health management approach to long term condition care:

  • Risk stratification based on NICE guidance
  • Prioritisation to optimise treatment early in those with greatest need
  • Deploying the wider workforce to support self-management and personalisation of care

The session will allow you to bring in your questions, case studies and other queries as we continue to improve your understanding of the SE CVD data and using the frameworks to identify and risk-stratify patients. 

Our expert panel will include:

  • Dr Matt Kearney – Executive Clinical Director for Cardiovascular Health, UCLPartners Academic Health Science Network. National Director Blood Pressure Optimisation Programme. Former National Clinical Director for Cardiovascular Disease Prevention, NHS England. General Practitioner Shropshire
  • Dr Richard Blakey – GPSI Cardiology, CVD Clinical Lead Kent Surrey & Sussex Academic Health Science Network, Hypertension Clinical Lead Sussex Health & Care Partnership ICB.

This event is open to all MDT colleagues in Primary Care across the South-East with an interest or role in CVD ; please feel free to forward this invitation to colleagues. For those who cannot join or have clinical commitments, the session will be recorded and made available with any resources that are also used during the session.
