Setting Up An Apprenticeship

Apprenticeship Agreement (Completion by employer)
You must sign an apprenticeship agreement with your apprentice at the start of their apprenticeship. This gives details of:
• the skill or occupation the apprentice is being trained for
• the name of the apprenticeship they are working towards
• the dates during which the apprenticeship is expected to take place
• the amount of off-the-job training they will receive
You can write your own apprenticeship agreement or download an apprenticeship agreement template.
Training Plan (formally commitment statement) (Sent by education provider for completion by employer, education provider and learner)
You must sign a training plan with your apprentice and education provider.
This sets out how you, your training provider and the apprentice will support the successful achievement of the apprenticeship, including through experience gained on the job.
You should make sure that the person in your organisation that is managing the apprentice on a day-to-day basis is aware of the commitments that have been made.
You can write your own or use the apprenticeship training plan template.
It must include:
• the planned content and schedule for training
• what is expected and offered by the employer, the training provider and the apprentice
• how to resolve queries or complaints
You should also make sure that your apprentices have confirmed their apprenticeship details on the My apprenticeship service.
The apprentice will only get their apprenticeship certificate after they have passed the assessments at the end of their study. This demonstrates they are occupationally competent.
Contract (Sent by Salisbury DPS and signed by employer and education provider)
This is the agreement between employer and HEI that they will provide the programme to learners – sent by Salisbury DPS after receiving the URN form.
Please do not sign provider contracts / service agreements if they send them direct. They must be in the NHS framework contract only.
URN Form (Completed by employers)
Order forms can be downloaded from the Order Forms section on the FutureNHS Workspace – https://future.nhs.uk/SalisburyApprenticeshipEF
You DON’T need to send an order form if the standard’s funding band is below £10k and it is a repeat enrolment – i.e. you have a previous framework order covering that standard.
DBS and Occupational Health Checks (Employers)
Please check with education provider whether this applies.
Clinical Apprenticeships:
You will need to put more in place for apprenticeships such as RNDA, SNA, AHP
Learning Environment Audit (Employers and education provider)
This may only apply to certain roles such RNDA/SNA . If you already have one of these for example with a HEI/FE College please feel free to share this with the education provider to save repetition of an additional audit. Please check with the education provider if you need one.
Placement audits (Employers and education provider)
Informs on the maintenance or improvements to the student practice placement experience through the provision of evidence of standard level met and highlights areas of potential concerns or agreed actions. Check with the education provider if an additional one is required.
