Developed primarily for GPs, these Action Learning Sets (ALS) are based on work undertaken in Yorkshire and Humber AHSN. They aim to help GPs understand the complex issues surrounding stopping inappropriate medicines safely and will also help PCNs deliver the Medicines Optimisation elements of the new GP QOF contract. HEE South are evaluating our ALS with a veiw to rolling out this methodology across England.
Delegates are required to attend all 3 sessions, review an e-learning tool and pariticipate in the evaluation.
Three half day sessions over 3 months:

- Session 2 – Shared decision making, evidence, tools
- Session 3 – Workshop and facilitated discussions to explore experiece and learning
Delegate criteria
Please note that the Polypharmacy ALS are aimed mainly at GPs.
Other non-medical prescribers especially pharmacists working in GP Practices and other who work with patients exposed to polypharmacy could be eligible e.g. Frailty Advanced Nurse practitioners.
For maximium benefit it is strongly recommended that delegates;
1) Must be working in primary care
2) Must be working with patients with multi morbidity and be responsible for the holistic care of such patients (i.e. not specialists in single long-term conditions)
3)Must be working with older patients including those deemed to be ‘frail’
