SUMMARY:CVD Prevention - UCL Partners: Proactive Care for Atrial Fibrillation
DESCRIPTION:UCLPartners is providing a series of 1-hour online education sessions for healthcare professionals involved in management of long-term conditions\, with a specific focus on cardiovascular disease. \nThese sessions will provide practical support for implementing the Proactive Care Frameworks and highlight high impact clinical interventions for specific conditions. This series will also identify opportunities to utilise the wider workforce in LTC management and include time for questions and discussion on each topic. \nThe Proactive Care Frameworks focus on how to do things differently at scale: they enable practices to prioritise clinical activity by stratifying patients who are at highest risk; they deploy the wider workforce to reduce the workload for GPs; and they improve the personalised care offer for patients. \nThese webinars are opened to all health care professionals involved in management of long-term conditions in primary care (GPs\, nurses pharmacists\, practice managers etc) and take place on the third Tuesday of the month. \n\nOur session on 18 April 2023 will focus on Proactive Care for Atrial Fibrillation: \n\n\nRisk stratification and optimisation for stroke prevention\nStroke and bleeding risk assessment\nDOACs vs warfarin\nSafe prescribing of anticoagulants and on-going monitoring\n\nRegister here the free online session: CVD Prevention – Supporting primary care to do things differently Tickets\, Tue 18 Apr 2023 at 13:30 | Eventbrite
