DESCRIPTION:Event: HEE FCP (MSK) Roadmap – Stage 1-A practical approach to mapping your Knowledge\, Skills & Attributes \nDate: Wednesday 21st July 2021 \nTime: 19.00-20.00 \n  \nPlease register for the event here: BNSSG TH FCP (MSP) stage 1 mapping \n  \n  \nA bit about the webinar… \n  \nSince commencing her training hub role\, Lizzie Bradshaw has been aware that there has been a certain degree of confusion (she includes herself in this) about the practical process of mapping to the Knowledge\, Skills & Attributes (KS&A) section within stage 1 of the HEE FCP MSK Roadmap. \n  \nUsing the FCP spreadsheet in the study tools section of the HEE Roadmap Landing Page\, Lizzie has mapped herself against stage 1 and following this\, has designed her PDP against stage 2. \n  \nLike many people going through this process\, we are learning as we go\, but we felt that it might be helpful to share this learning in a practical way to aim to help others from practitioners to supervisors.  \n  \nKerri Magnus\, ACP lead will chair the meeting discussion. Rachel Butt (HEE Roadmap Supervisor) will run through the mapping of Lizzie Bradshaw’s (MSK FCP) personal KS&A to discuss examples of potential evidence for those on the portfolio route and also demonstrate the use of the HEE FCP framework spreadsheet tool to record this portfolio of evidence. \n  \nWe will discuss the process\, time taken and projected time to complete Stage 2\, how to use the tool and then map 2-3 items of evidence in each domain (A-D) to give examples\, leaving time for questions at the end. \n  \nAlthough this is structured around the MSK domains the practical tools and process will be relevant to the Paramedic one and in the future we are replicating this webinar for the Paramedics. \n  \nPlease join The BNSSG Training hub in our a webinar on Weds 21st July at 7pm-8pm to share this learning. \n  \nNB: This event will be recorded and uploaded to the BNSSG Training Hub website \n  \n www.bnssgtraininghub.com \n  \nPlease email kerri.magnus2@nhs.net for further information.

DESCRIPTION:A webinar series facilitated by the National Association of Primary Care (NAPC) \nDelivered virtually\n\n\n\n\n\n\nThe NHS Long Term Plan set out a widely supported route map to tackle our greatest health challenges\, from improving cancer care to transforming mental health\, from giving young people a healthy start in life to closing the gaps in health inequalities in communities\, and enabling people to look after their own health and wellbeing. \n\n\n\n\n\n\n\nThe aim of this webinar series is to bring together colleagues from across the South West to have conversations on ‘Tackling Health Inequalities through a simple approach to population health management’ and to build relationships across the region. \nTo ensure the conversation is as rich as possible\, we invite colleagues from across the system\, as well as those in primary care\, to support ICS’s to collectively address the right priorities for their residents using their collective resources wisely. \nBy the end of each webinar\, participants should: \n\nBe reminded of the social determinants of health and wellbeing\nHave a base level understanding of population health management – an increasingly important concept behind new care model development\nUnderstand how population health management is key to tackling health inequalities\n\n  \nTarget audience \nAll colleagues across the South West region \nDate\, time and booking \n1 July 2021\, 12:30-14:30 – Webinar Registration Link \nFacilitators – National Association of Primary Care \nThe facilitators for these sessions are Dr Nav Chana\, Dr Leena Patel\, Annabel Beasley and Andy Mullins\, who will be providing a practical insight into population health management and managing health inequalities. \nThe National Association of Primary Care (NAPC) is a not-for-profit\, social purpose development and support organisation.  Their mission is to improve the health and wellbeing of defined populations locally and nationally.  The organisation is led by serving health care professionals\, which grounds them with practical experience and gives them real credibility as they work with primary health and care teams. \nNAPC were responsible for developing the original Primary Care Home (PCH) model\, that brings together a range of health and care professionals\, to provide enhanced personalised \nand preventative care for their local community\, based on their specific health and wellbeing needs.  The PCH Programme\, led by NAPC and supported by NHSE\, has gathered huge momentum since its inception in autumn 2015. From 15 original rapid test sites – there are now more than 240 sites across England\, covering 11 million patients 17% of the population. \nPrimary Care Homes are well established and mature versions of Primary Care Networks (PCNs) – the cornerstone of the new service model featured in the NHS Long Term Plan which was published in January 2019.  Since 2015\, NAPC has facilitated the development of primary health care across 5 ICSs and 14 STPs covering over 350 PCNs. \nCancellations \nWhen partnering with development providers\, a fee is paid for their services\, and although this fee is not passed on to you\, by registering you will be signing up to a place that somebody else could fill.
