SUMMARY:Improving population health through person centred digital care
DESCRIPTION:WED 08 September 12.30 – 13.45  \nImproving population health through person centred digital care \nThe principle and practice of population health management is now well established in policy and practice\, and at both system and primary care network (PCN)\, understanding and responding to your population’s health needs is critical to the future success and quality in the NHS care. \n  \nGroup clinics are emerging as a powerful tool to drive population health improvement\, with exemplars in both primary and specialist care settings. There is already a strong evidence base (mainly from face to face group clinics) to support this new way of working. \nThis session brings together those who have applied group clinics and those focused on improving population health in shared virtual space to share experiences and reflect together on: \n\nThe opportunities and challenges of using group clinic models to improve population health\nWhat needs to happen to support teams to use population health data to inform group clinic model development\n\n  \nFollowing the session\, we will produce a top tips paper to support the planning of population health focused group clinic models. \n  \nBook here: https://www.eventbrite.co.uk/e/149839968379  \n

SUMMARY:HEE FCP (MSK) Roadmap - Stage 1-A practical approach to mapping your Knowledge\, Skills & Attributes
DESCRIPTION:Event: HEE FCP (MSK) Roadmap – Stage 1-A practical approach to mapping your Knowledge\, Skills & Attributes \nDate: Wednesday 21st July 2021 \nTime: 19.00-20.00 \n  \nPlease register for the event here: BNSSG TH FCP (MSP) stage 1 mapping \n  \n  \nA bit about the webinar… \n  \nSince commencing her training hub role\, Lizzie Bradshaw has been aware that there has been a certain degree of confusion (she includes herself in this) about the practical process of mapping to the Knowledge\, Skills & Attributes (KS&A) section within stage 1 of the HEE FCP MSK Roadmap. \n  \nUsing the FCP spreadsheet in the study tools section of the HEE Roadmap Landing Page\, Lizzie has mapped herself against stage 1 and following this\, has designed her PDP against stage 2. \n  \nLike many people going through this process\, we are learning as we go\, but we felt that it might be helpful to share this learning in a practical way to aim to help others from practitioners to supervisors.  \n  \nKerri Magnus\, ACP lead will chair the meeting discussion. Rachel Butt (HEE Roadmap Supervisor) will run through the mapping of Lizzie Bradshaw’s (MSK FCP) personal KS&A to discuss examples of potential evidence for those on the portfolio route and also demonstrate the use of the HEE FCP framework spreadsheet tool to record this portfolio of evidence. \n  \nWe will discuss the process\, time taken and projected time to complete Stage 2\, how to use the tool and then map 2-3 items of evidence in each domain (A-D) to give examples\, leaving time for questions at the end. \n  \nAlthough this is structured around the MSK domains the practical tools and process will be relevant to the Paramedic one and in the future we are replicating this webinar for the Paramedics. \n  \nPlease join The BNSSG Training hub in our a webinar on Weds 21st July at 7pm-8pm to share this learning. \n  \nNB: This event will be recorded and uploaded to the BNSSG Training Hub website \n  \n www.bnssgtraininghub.com \n  \nPlease email kerri.magnus2@nhs.net for further information.

SUMMARY:Person centred digital care: what is it and how can we support its development?
DESCRIPTION:WED 14 July 12.30 – 13.45  Person centred digital care: what is it and how can we support its development? \nThe principle of person centred care and personalisation is now well established in policy and practice and is critical to the future success and quality of NHS services. With COVID 19 forcing many services to move online\, what does this mean for person centred practice development and what do system leaders need to do to support clinical teams and the personalised care workforce to make the shift successfully? This session brings together those who have been working to shift person-centred care and support online over the last 12 months and provides a safe space to share experiences and reflect together on: \n\nThe opportunities and challenges of delivering person centred digital care\nWhat needs to happen to support the workforce to maintain person centred care in the virtual world\n\nFollowing the session\, we will produce a summary paper to support system leaders to plan to support person centred digital care in 2021 and beyond. \nBook here:  https://www.eventbrite.co.uk/e/148039394815
