East Bournemouth (South Coast Medical) Population Health/CCLIP PLT Session

The GP locality chairs and Dr Claire Lenham, as Clinical Lead for Primary Care, wish to extend the invitation to all GPs in Dorset, including salaried and locum GPs, to take part in the forthcoming learning events looking at population health.
 

Each locality will be holding a 3-hour workshop to consider the health of their local population, considering the data and their knowledge of the population, supported by members of the CCG Primary Care team and Public Health Dorset locality link workers to identify an area of focus for 2019/20; and then decide how they may wish to address this area, considering both prevention and also management of long term conditions.

They really wished to ensure that all GPs may have the opportunity to engage in this. It may also provide useful evidence for GP appraisal for quality improvement activity too!
