Diabetes Education Session

Session information
Delivered by clinical leads in Diabetes and Familial Hyperlipidaemia, this session will explore how we can provide better support to diagnose FH, prevent diabetes and recognise and support those at greater risk in our communities, through the use of the tools and data we have access to. This session has been designed to support the wider Diabetes programme of work in Dorset, including the Diabetes Prevention Programme (DPP), the arrangements for CCLIP and the goal of improving HbA1c control, blood pressure and cholesterol in ‘amber’ patients via MDTs, which is being taken forward via the Wessex Diabetes Transformation Programme. The session will be run as an interactive workshop with focus on real data from your community, providing opportunities to discuss and learn together.
In Dorset, this fits well with work under way in diabetes, particularly via the CCLIP which focuses already on using PRIMIS to search patient databases to identify patients who are not currently meeting their clinical targets for the 3 national treatment targets and to develop appropriate management and support plans.
Who should attend?
To get the most from the session we would recommend a GP, Practice Nurse and someone from the practice who is involved in running the GRASP tool should attend together. Where this is not practicable, attendance from any 2 of these 3 would be beneficial.
Before attending:
In advance of attending the session attendees will need to run their practice FH and PRIMIS diabetes tools for CCLIP and email the summaries to primarycare.workforcecentre@dorsetccg.nhs.uk before 18th June 2018. This will be used to produce handouts for you to use during the session enabling you to focus on your community.
