Beyond Glycaemic Control; Getting to the HEART of Diabetes Management

We are excited to share with you below the first in a series of CVD education webinars, that will run sessions on a range of CVD topics over the next year, delivered by clinicians at Cardiometabology.  The webinars are free to attend and will be available to all MDT colleagues with an interest in CVD to register from Primary, Community and Secondary Care across the South East. The events will run live for people to attend and have the opportunity to engage in Q&A sessions and after the event the recordings will be available and shared as a link for anyone that missed it.

Join us for Beyond glycaemic control getting to the heart of diabetes. For the session we’ll give an introduction to the relationships between diabetes and cardiovascular disease. We’ll then describe how using an awareness of these relationships we designed a cardiometabolic diabetes pathway and created an award winning cardiometabolic diabetes service in Kent and Medway. We’ll then see the significance of this model and how any allied healthcare professional can realise their role in future diabetes care.

Using the above we will then describe a practical patient clinician journey from the primary and secondary care perspective. We’ll then do a recap of the ADA/EASD consensus statement 2019 and then moving onto our two case studies.

The session is focused around the management of patients with diabetes and established cardiovascular disease. The key discussions will be around the multi-factorial management of patients with co-morbidities. Optimising care and making every consultation count.
